The First Visit


  • This is a 60-90 minute meeting, before which your doctor reviews your medical record in detail. We will discuss your current state of health, your wishes, and your priorities. Together, we will craft a wellness vision. This conversation will be followed by a comprehensive physical exam.

LIFESTYLE 360 VISIT

Each year after your initial intake, you’ll receive a Lifestyle 360 visit—an upgraded version of the annual wellness visit you're used to. This comprehensive appointment includes a full physical exam, routine screenings, and preventive care, along with expanded assessments to support whole-person health.



Your Lifestyle 360 visit includes:

  • Physical exam and standard screenings
  • EKG and spirometry (lung function testing)
  • Body composition analysis to support strength and prevent frailty
  • Vision screening
  • Advanced cardiac risk panel
  • Mental and cognitive health assessments
  • Long-term wellness planning tailored to your goals

To support your mind-body connection, we also include a 12-class pass to Mountain Path Wellness, conveniently located upstairs, for 1:1 tai chi or qigong sessions with a certified instructor.

Chronic Care Management

Stay in touch and make headway on chronic disease management. Through a series of phone calls every 1-2 weeks, you and your doctor keep on top of things like diabetes, high blood pressure, anxiety, congestive heart failure, COPD and other diagnoses that can follow a person through life. Together, we use small corrections to take control of your health.

Remote Patient Monitoring

RPM is another Medicare-covered method for keeping an eye on the numbers that matter in chronic disease management: weight, blood pressure, blood sugar, etc. You log the numbers on intuitive and state-of-the-art tech. Your doctor then reviews that data weekly and makes adjustments that matter, all without leaving the comfort of your home!

Acute visits

Life happens! Earaches, asthma exacerbations, a funny looking spot, new and unexplained symptoms, stitches, joint injections, diagnostic procedures, or wound care. These are what sick visits are for. Appointments with us are self-scheduled: no phone tree, no waiting on hold. We offer same-day, next-day, and urgent visits both in-person and via telehealth.

Advanced Care Planning

We discuss priorities and longterm goals with patients facing progressive diagnoses or advancing frailty. In the comfort of the office, you and family can engage in meaningful discussion about your wishes and what matters to you.

End of Life Care

When patients are approaching the end of life, we are there to guide them and their family through the process in the comfort of home, if desired.
